To successfully request patient care report, please contact us in one of the following ways:

• Fax all necessary documents to (740) 385-8439
• Mail all necessary documents to:

• In person at the following location:

    • Must have a valid photo Id / other Pertinent Identification
    • Any necessary documents
• Attorney and Insurance Company Requests
  • All requests will be forwarded to Ambulance Medical Billing (AMB) for completion.
